San Diego Outreach Navigator (full-time position)

Military Transition Support Projectís (MTSP) zero8hundred program is a fiscally sponsored project of Mission Edge San Diego. zero8hundred is an innovative collaboration among San Diego's military, government agencies, elected officials, nonprofits, businesses, and philanthropic institutions. Together, these organizations have developed a plan to better coordinate community resources for transitioning Active Duty Military, Veterans, Guard, reserve and their families staying in San Diego, creating a system that will directly connect service members to critical transition resources as they exit the military. The services coordinated and offered under this program include job placement, education, and social services Ė all part of a holistic veteran wellness model.

 

Reporting to the Program Director, the Peer Navigator is responsible for program outreach on military bases and other programs as well as the provision of direct case management services and support to veterans and their families. The Peer Navigator will provide support to individuals through telephone/email and in-person meetings to assist transitioning Active Duty Military, Veterans, Guard, reserve and their families in their transition from military to civilian life. Areas of support include education and jobs, mental and physical health, basic needs and personal connections. zero8hundred provides supportive counseling and mentoring services, while referring veterans to trusted service providers in these critical areas. Follow-up support from the zero8hundred staff will ensure services are delivered in an effective and coordinated fashion. This position also has significant outreach and administrative support, and some supervisory responsibilities outlined below.

 

The Peer Navigator will work with a team of Peer Navigator MSWs who are employed at zero8hundred and interns from Master of Social Work programs at accredited universities. All peer Navigators provide supportive counseling, evaluation and referral services, and case management.

 

The hours for this full-time peer Navigator position are expected to be from 9:00 to 5:30 pm four of the days Monday through Friday, with the fifth dayís hours from 11am-7:30pm. These days/hours may be adjusted by the zero8hundred staff. The work location for this position is expected to be 4420 Hotel Circle Court, Suite 250, San Diego CA 92108.

 

Key Duties and Responsibilities

 

Participant Case Management (50% position focus):

 

  • Build relationships with transitioning veterans and family members who have registered for zero8hundred services.
  • Assist each zero8hundred client in critical areas such as employment, education, housing, behavioral and primary healthcare, legal services, financial needs and any other support that may be required.
  • Refer participants to zero8hundred-approved service providers in these areas, while continuing to monitor progress toward key outcomes.
  • Remain connected with zero8hundred participants throughout the entire process, conducting follow-up calls, meetings and visits as required to ensure all aspects of the participantís transition plan are executed.
  • Continue to check in frequently (following zero8hundredís protocol, and as scheduled with client) to ensure the wellbeing of military and veteran participants and their families.
  • Record all client information in the zero8hundred case management database for follow-up and reporting.

 

Other Duties and Responsibilities (50% position focus):

Become the zero8hundred subject matter expert in resources such as employment support, educational/vocational opportunities, physical and behavioral health, basic needs, and social/peer connections.

  • Outreach responsibilities:
  1. Represent zero8hundred including public speaking and making presentations at a wide variety of on and off-base venues, including career and transition fairs and transition classes. Work professionally with transitioning military personnel and spouses with the goal of having eligible members opt into the zero8hundred program. Build positive relationships with stakeholders from other organizations including representatives of the armed forces.  
  2. Represent zero8hundred at partner and collaborative meetings as assigned. Maintain and develop effective working relationships with veteran services organizations.
  • Supervise a team of MSW interns: provide oversight on client contact and individual spreadsheets quarterly within a semester, and weekly transfer collected data to the program spreadsheet
    • Communicate closely with the Program Director on evaluation of internsí notes and spreadsheets.
    • Administrative responsibilities: support the Program Director and Executive Director with day-to-day program operations and administration. Complete high quality staff work to further project goals.
      • Participate in internal and external committees and staff meetings as needed.
      • Encourage open expression of ideas and opinions for staff development and zero8hundred program improvement.
      • Continue to learn about issues, resources and opportunities related to military veterans and their families.
      • Consult with clients, social workers, case managers, VA personnel, peer Navigators, and the Program Director when clinical concerns arise. Interaction may be by phone, email, in person or through other means.
      • Work responsibly in what will be a semi-independent work environment at times; communicate regularly with Program Director and submit progress updates.
      • Observe confidentiality, privacy, and dignity of each client.
      • Stay current with and adhere to all requirements established by the California Board of Behavioral Sciences (BBS) and National Association of Social Workers (NASW) Code of Ethics.

Knowledge, Skills, and Abilities

 

  • MUST possess Masterís in Social Work (MSW) from an accredited university; current Associate Clinical Social Work (ACSW) registration/certificate (or immediate eligibility) required. Licensed Clinical Social Work (LCSW) certificate not required.
  • MUST possess previous military service, or experience as military spouse or dependent.
  • MUST have a valid driverís license, current car insurance and reliable transportation
  • Strong experience with Excel spreadsheets
  • Experience developing positive, professional working relationships across a variety of organizations, including, but not limited to, DOD, non-profits, and employers, with an ability to promote a unified, collaborative approach to care within these agencies.
  • Experience working with veterans and active duty military service members and families strongly preferred
  • Experience and comfort with public speaking and making presentations.
  • Strong written and verbal communication skills
  • Experience using MS Office, database, and case management software.

Physical Requirements and Work Environment: Work is typically performed in an office setting; employee regularly sits at a computer station and operates electronic equipment; occasionally lifts, carries and positions objects weighing up to 20 pounds. Position involves regular use of a computer and keyboard, telephone, and face-to-face communications; employee should be able to communicate clearly and professionally in all of these manners. The employee may spend periods of time sitting, standing, walking and driving including ride sharing, possible long distances. Compensation: The annual salary for this full-time, exempt position is $48,000. Company benefits include paid time off, medical benefits, a company retirement plan, business mileage reimbursement and clinical supervision. 

 

Additional Information: This position requires a criminal background check prior to hire. No relocation offered.
